RN - Hospice Case Manager

Overview:
The RN Hospice Case Manager is responsible for managing; the overall plan of care, care coordination with members of the team and caregivers and outcomes for assigned patients using critical thinking skills.
Responsibilities:
- Utilize the nursing process to structure care and individualized patient goals
- Demonstrate ability to assess and interpret clinical findings, verbal and non-verbal communication
- Demonstrate knowledge in performing routine nursing skills
- Demonstrate knowledge and expertise in performing technical skills
- Assess and document patient’s response to treatment
- Administer medications according to the organizations policy
- Develop licensed nursing assistant plan of care, and revises as needed
- Implements physician orders accurately
- Regularly participate in interdisciplinary case conference and document patient information as needed
- Document patient care practices and information regarding patient status according to agency policy
- Knowledge and implementation of POC, evaluating effectiveness of care and progress toward patient goals and regularly update POC as needed
- Clinical decision making using critical thinking/problem solving skills based upon best practices
- Provide case related information to physicians, team leaders, supervisors and ancillary members of the team as required
- Provide patient centered care and education that is goal driven to maximize discharge potential; involve patient/family in self-care as appropriate
- Delegate non-nursing tasks to support personnel
- Delegate appropriate tasks/assignments to LPN’s
- Provide clinical supervision of non-nursing staff
- Comply with regulatory information
- Demonstrate self-assertiveness by keeping up-to-date with required skills and practice, and identify and seek opportunities to learn unfamiliar skills
- Demonstrate patient advocacy and empowerment by their actions and communication
- Participate in education on quality and patient outcomes
- Responsible for accurate OASIS assessment and documentation at all necessary points of care
- Maintain a productivity average of 4.5 visits a day including completion of all necessary and accompanying documentation within 24 hours
- Arrive on time for meetings/education
- Practice nursing within the scope of license set by the NH Board of Nursing
- Engages in respectful communication (both verbal and written) with team members, patients, family members, and the community
- Adheres to agency wide policies
- Other duties as assigned
Qualifications:
- Current NH RN license
- Minimum 2-5 years direct care experience preferred
- IV Skills preferred
- Excellent communication skills necessary (verbal and written)
- Telephone triage experience preferred
