Duties Highlights for an Admission Registered Nurse (RN):

• Provide high level, skilled nursing care to patients
• Provide meaningful education to patients
• Support the RN Case Manager
• Execute tailored Care Plans in the home setting
• Communicate status changes
• Work through plan of care adjustments as patients move toward reaching health care goals

Requirements for an Admission Registered Nurse (RN):

• Active NH RN License
• Homecare experience strongly preferred
